Treatments
How a comprehensive duplex ultrasound evaluation uncovers the true source of leg symptoms, guides precise treatment decisions, and determines what can be preserved.
A vein evaluation is not simply a screening. It is the foundation on which every
treatment decision at The Vein Clinic of Dallas is built. Dr. Husein Poonawala a Harvard-trained vascular and interventional radiologist with double board
certification in venous and lymphatic medicine, personally directs every
evaluation. No treatment is recommended before the full picture is clearly
understood.
Vein disease is frequently underestimated because its most significant activity
is invisible at the surface. Legs that feel heavy by evening, ankles that swell
progressively through the day, aching that worsens after standing, restless legs
at night, and cramping that disrupts sleep are all common early signs of chronic venous insufficiency , a condition in which damaged vein valves allow blood to pool in the lower legs rather than circulate efficiently back toward the heart.
What appears on the surface, whether as visible varicose veins, spider veins, or skin changes, often reflects a more complex pattern of venous dysfunction beneath it. A thorough evaluation maps that full picture. It also identifies what does not need treatment: some veins that appear compromised are compensating adequately and are better monitored over time than intervened upon.

A vein evaluation is indicated when symptoms suggest venous insufficiency, or when
visible changes in the legs raise questions about what is happening beneath the surface.
Leg heaviness that builds throughout the day and worsens toward evening, a consistent early symptom of venous insufficiency
Ankle or lower leg swelling that worsens as the day progresses and does not fully resolve overnight
Aching, throbbing, or discomfort in the legs that worsens after standing or prolonged sitting
Visible varicose veins or spider veins, whether symptomatic or not, that warrant a proper assessment of their source
Restless legs at night or leg cramping disrupting sleep, which can be linked to venous insufficiency in a meaningful subset of patients
Skin changes such as discoloration, thickening, or itching in the lower legs that may indicate advanced venous disease
At The Vein Clinic of Dallas, our diagnostic approach goes well beyond a basic vein
screening. Every evaluation combines multiple assessment tools to build a complete
picture of your venous anatomy and function.
Every evaluation begins with a thorough history and physical examination, performed personally by Dr. Poonawala. During this initial consultation, he reviews your symptoms, medical history, risk factors, and prior treatments in detail. As part of the history and physical, Dr. Poonawala also performs a focused, limited bedside ultrasound to get a preliminary look at your venous system, confirm whether reflux is present, and assess whether your symptoms align with what he observes clinically. This is not the formal diagnostic mapping study; it is a physician-performed clinical tool that guides the next step in your care.
By evaluating how blood moves while you are standing and moving, rather than at rest, we capture functional venous behavior that static imaging alone cannot reveal. This identifies reflux patterns and tributary involvement that might otherwise be missed.
When the history, physical, and initial bedside assessment confirm that venous reflux is present and treatment is likely indicated, patients proceed to a formal diagnostic duplex ultrasound and venous mapping study. This comprehensive study precisely identifies which veins are malfunctioning, their caliber, their relationship to surrounding anatomy, and the most appropriate treatment approach for each. Venous mapping may take place on the same day as the initial consultation or be scheduled at a separate visit depending on the clinical picture and scheduling.
Supplementary tools that visualize abnormal surface vein patterns and help confirm the connections between spider veins, varicose veins, and deeper refluxing vessels.
Dr. Poonawala personally reviews and interprets every finding with you, explaining what is and is not clinically significant and discussing your options and next steps in detail. The consultation takes approximately 20 to 25 minutes.

Every evaluation is personally directed by Dr. Poonawala. No treatment recommendation is made without direct physician oversight. Your evaluation is never rushed or delegated.
Duplex ultrasound, flow dynamics assessment, venous mapping, thermal vascular imaging, and vein lights combine to build a complete picture that simpler screening approaches miss.
We take the time to understand your venous system fully before making any recommendation. If treatment is not indicated, we will say so clearly. The goal is your vein health, not your procedure count.
Your vein evaluation is not a one-time event. Annual follow-up evaluations allow us to monitor your vein health over time, detect early recurrence, and protect the results of any treatment we perform together.
If your legs feel heavy by evening, swell around the ankles, ache after standing, or keep
you awake at night with cramping or restlessness, a comprehensive vein evaluation can
identify what is driving those symptoms and what, if anything, genuinely needs to be done
about it.
