The hidden cause of persistent leg swelling, and how we help identify and manage it
Lymphedema is a condition where the lymphatic system becomes impaired, leading to chronic leg swelling that does not easily resolve. Many patients are told their swelling is simply related to aging or fluid retention, but persistent leg swelling often has an underlying cause that can be identified and properly managed. At The Vein Clinic of Dallas, we start by determining exactly what is driving your swelling before assuming all swelling is the same.
Lymphedema frequently develops alongside or after years of unmanaged vein disease. When failing veins create excess pressure in the legs, the lymphatic system becomes overwhelmed over time. This combination is sometimes called phlebolymphedema, and it is far more common than pure lymphedema alone. Identifying the venous component is critical because treating vein
disease first can significantly reduce swelling and improve lymphatic function.
The lymphatic system helps drain excess fluid from tissues and return it to the bloodstream. When this system becomes damaged, overwhelmed, or blocked, fluid builds up in the tissue, causing persistent swelling. Primary lymphedema is a rare genetic or developmental condition where the lymphatic system is underdeveloped. Secondary lymphedema is far more common,
occurring when the lymphatic system is damaged by chronic venous, vein disease left untreated for too long, surgery, trauma, infection, or cancer treatment such as lymph node removal.
A note on lipedema: lipedema is a separate condition involving abnormal fat distribution in the legs, often confused with lymphedema or general swelling.
It is not caused by vein disease, though it can coexist with venous or lymphatic dysfunction. A proper evaluation helps distinguish between these conditions.
Swelling is worst at the end of the day and often does not fully resolve with leg elevation
Phlebolymphedema occurs when vein disease overloads the lymphatic system. This is a common and treatable combination
Treating the venous component first often results in significant reduction in swelling and improved lymphatic drainage
Many treatments including pneumatic compression devices and vein procedures are covered by insurance when medically necessary
Swelling that is often worse at the end of the day and does not fully resolve overnight or with simple elevation.
A persistent sense of weight, fullness, or tightness in the lower legs that builds through the day and impairs daily activity.
Unlike simple edema, lymphedema- related swelling persists even after extended rest with legs elevated.
Progressive skin changes including a firmer, thicker texture that develops as chronic swelling goes unmanaged.
In more advanced cases, skin takes on a dimpled, folded, or woody character that indicates long-standing lymphatic impairment.
Recurring cellulitis or skin infections can be a sign of impaired lymphatic drainage. The lymphatic system plays a key role in immune defense of the skin
Why Early Evaluation Matters
Without proper identification and management, lymphedema typically progresses and
complications accumulate. Early evaluation prevents much of this.
Untreated lymphedema typically progresses, with swelling becoming more severe, more constant, and harder to manage with conservative measures.
Long-standing swelling causes progressive skin thickening, fibrosis, and texture changes that become increasingly difficult to reverse.
Impaired lymphatic drainage compromises skin immunity, dramatically increasing the frequency and severity of bacterial skin infections.
Progressive swelling, heaviness, discomfort, and skin changes progressively limit mobility, comfort, and participation in daily activities.
When a venous cause is present and goes untreated, vein disease continues to progress, causing greater lymphatic overload and worsening the overall picture.
In advanced phlebolymphedema, the combination of venous and lymphatic failure can lead to skin breakdown and venous ulcer formation.
Management starts with a thorough evaluation to determine whether your swelling is due to vein disease, lymphatic dysfunction, or both. Our focus is on identifying exactly what is driving your symptoms rather than assuming all swelling is the same.
If vein disease is present, we treat it first. Correcting venous insufficiency reduces pressure in the legs and often results in significant improvement in swelling and lymphatic function. For true or residual lymphedema, we coordinate a structured management plan that may include lymphedema decongestive therapy, pneumatic compression devices, properly fitted compression garments, and long-term vein health maintenance.
Correcting abnormal venous reflux reduces the elevated pressure driving fluid into surrounding tissue, addressing a primary contributor to lymphedema.
A structured program combining manual lymphatic drainage, compression bandaging and therapeutic exercise to reduce limb volume and improve tissue health.
At-home pump therapy organized through our clinic for qualifying patients. Significantly improves lymphatic flow and reduces swelling when used consistently.
Properly fitted graduated compression garments as part of a comprehensive swelling management plan.
Education on skin care, swelling control, and lifestyle strategies to maintain leg health over time.
Reduction in leg and ankle swelling over time as venous and lymphatic function improves
Less discomfort, tightness, and heaviness in the lower legs throughout the day
Prevention of worsening skin changes, recurring infections, and further complications
Better comfort, mobility, and quality of life with a long-term plan tailored to your needs
Plans accepted include BCBS, Anthem, Medicare, Aetna, Cigna, United Healthcare and more.
Every evaluation is performed by Dr. Husein Poonawala. Nothing is delegated. You receive expert, direct physician attention at every visit.
We determine whether your swelling is due to vein disease, lymphatic dysfunction, or both, using clinical examination and vascular ultrasound to guide our findings precisely.
If vein disease is part of the problem, we treat it first to relieve pressure and improve lymphatic drainage, addressing the root cause rather than managing symptoms alone.
We organize lymphatic drainage and compression therapy and guide ongoing management with a plan tailored to your specific needs and goals.
If you are struggling with persistent leg swelling, heaviness, tightness, or discomfort,
whether or not you have had prior vein treatment, you may have lymphedema, vein
disease, or both. If your swelling has not improved, keeps coming back, or remains
unexplained, a proper evaluation is the next step.
